Credit Card Authorization Form
We require all of our rental accounts to have a valid credit card on file. This form is for enrolling your credit card.
Rental Contract
Fill out all of your personal information and requested instrument, then read the terms, put Y or N for the loss and damage waiver, initial, sign and date. JKEAA staff will be happy to fill in the prices, contract date info, and instrument details.
